1. ACKNOWLEDGMENT OF RISKS
As the legal guardian of the participant, I acknowledge that the practice of Jiu-Jitsu involves inherent risks
— including but not limited to physical injuries such as bruises, sprains, fractures, and, in rare cases,
more serious injuries — and that these risks exist despite all reasonable safety precautions taken by the
academy.

2. ASSUMPTION OF RISK AND LIABILITY RELEASE
In consideration of my child’s participation in the Summer Camp, I voluntarily assume all risks and,
except in cases of gross negligence or willful misconduct, I release the academy, its instructors, staff, and
affiliates from any liability for injuries or damages arising from participation.

3. EMERGENCY MEDICAL AUTHORIZATION
I authorize the academy to obtain emergency medical assistance for my child if needed (including first
aid, transportation, and treatment), and I agree to bear all related costs.

4. CALIFORNIA CIVIL CODE REFERENCE (SECTION 1542)
This release is intended to be as broad and inclusive as permitted under California law. I understand that
under Section 1542 of the California Civil Code, I may be waiving rights to claims I am not aware of at
the time of signing this release.

5. USE OF PHOTO/VIDEO
By signing this document, I authorize the use of photos or videos of my child for promotional purposes
by the academy (e.g., social media, website).

6. FINAL DECLARATION
By checking the sign box on the registration form, I declare that I have read, understood, and agreed to all of the above terms, and I affirm that I do so voluntarily and freely.
